Childcare Booking Form
By completing and submitting this form you are formally applying for Karen’s Childcare to provide childminding services for your wedding or event.
A deposit will be required on booking (non-refundable) to secure your date. with the remaining balance to be paid four weeks before the event.
In the case of hotel room childcare more than 3 children can be considered, however this would incur the cost of an assistant carer and possibly an additional hotel room.
Health & Safety is foremost. The contact numbers provided will be the contacts we will use on the event if we need to contact the child’s parent/carer.
Parental/Carer consent for each child must be completed and signed prior to the event and returned to us.
